Mental health support services in Bath and North East Somerset (BaNES)

We offer a range of community mental health support services for people aged 16+ who live in Bath and North East Somerset (BaNES) or who are registered with a BaNES GP.

Access Community Mental Health Team

Our Access Community Mental Health team helps people access the right mental health support when they need it.

Working alongside you, our experienced team will help you:

  • Identify your goals and help you take steps to achieve them
  • Access local mental health services, such as wellbeing groups or counselling
  • Access resources available to you in your local community
  • Develop coping technique to boost your resilience
  • Plan your next steps, focusing on building your independence and long-term wellbeing

This service is available to anyone aged 16+ living in Bath and North East Somerset or registered with a BaNES GP. Refer yourself or someone else by filling in our referral forms or contacting us using the details below.

Intensive Outreach Service

The Intensive Outreach Service provides personalised support to help people transition from in-patient mental health care back into the community, and provides intensive support to people in their own homes.

We take a person-centred, trauma-informed approach, including a step up and step down service.

This service is open to referrals via Avon and Wiltshire Mental Health Partnership Trust (AWP). To make a referral or have a discussion, please contact us using the details below.

SMI Physical Health Checks Outreach

SMI Physical Health Checks are available to people in Bath and North East Somerset on the Serious Mental Illness Register. Our team helps people access physical health checks and health improvement plans, any follow up appointments, and to develop their personal health improvement plans.
